does anybody have any idea what title to search for on eBay to get wheels like these? I know that the axle bolt is 15 mm diameter.

Practically any chair with 9 or 10 inch casters.

That setup looks extremely rough. I mean the way its done not the condition its in.

But that axle looks smaller than 15mm. Usually they are 12. With an m8 bolt. But who knows on a single sided caster.

They are the same as the ones sunrise, or handicare or puma etc use. Probably more. But they are all pretty interchangable with different bearings or spacers as needed regardless.

No idea but I would think so. Look 2 posts up I found you a few.

If not modify as needed. Or order a set of forks too and fit those. Its all mind over metal. Everything fits!

There may be diferences between the puma and the R2 ones that I searched for. But only minor ones. May need different bearings or spacers or whatever. Nothing that cannot be beaten into submission.

Theres nothing actually wrong with your existing ones. Take the forks to be sand blasted and painted. They will come back as new.
Strip the wheels and repaint them and they too will be better than new. Fit new stainless bolts, bearings and tyres etc and it will work better than new.
